Correction check·April 28, 2026

On April 28 th , 2026, at 9:30 AM, Licensing Program Analyst (LPA) David Nguyen and Licensing Program Manager (LPM) Chayntel Hunter met with Facility Representative/Site Supervisor, Guadalupe Espinoza for the purpose of a Proof-of-Corrections Inspection. LPA Nguyen and LPM Hunter toured the facility and reviewed records. There were eleven (11) children present supervised by three (3) staff during today’s proof-of-correction inspection. On 4/17/2026, the facility was cited two Type-A deficiencies for not having adequate supplies in children’s bathrooms, and failure to maintain play equipment, fixtures, furniture, and dwelling in good repai r . Furthermore, the facility was cited two (2) Type-B deficiencies for failure to keep yards free from hazards, broken equipment, and not having adequate amount of woodchips under the play structures. During today’s inspection, LPA Nguyen observed children’s bathrooms with adequate supplies. LPA also observed that fence gates were repaired. LPA observed outdoor activity space has been cleaned and organized. LPA observed broken and/or unsafe equipment has been removed and/or repaired. LPA received and reviewed children’s records and observed Acknowledgement of Receipt of Licensing Report (LIC 9224) signed by parents in children’s files. The deficiencies cited on 4/17/2026 are cleared with today’s visit. A Proof of Correction Letters were provided. In the areas that were evaluated today, no deficiencies were observed. An exit interview was conducted, and the report was reviewed with facility representative/site supervisor, Guadalupe Espinoza, and a Notice of Site Visit was posted by LPA and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
