What the inspector wrote

On April 28th, 2026, Licensing Program Analyst (LPA) Sarah Tibbett conducted a visit at the facility and met with Licensee Graciela Terrazas, for the purpose of making the master bedroom and bathroom on limits. During the visit, Licensee stated that children use the master bathroom when they can not use the hallway bathroom. The master bathroom is being used prior to department approval. LPA toured the master bathroom and observed that children must walk through the master bedroom to gain access. During the visit, LPA recommended adding child locks in the master bathroom, and determined the master bedroom and bathroom were safe for children. Master bedroom and bathroom usage is approved effective today 4/28/26. LPA Tibbett informed Licensee, Graciela Terrazas, that this report dated 4/28/2026, documents 1 Type-B citation during today’s inspection. An exit interview was conducted. Appeal rights were given and a notice of site visit was posted by the LPA and must remain posted for 30 days.
