What the inspector wrote

On February 17, 2026, Licensing Program Analyst (LPA) Stacey Williams met with Facility Representative, Malathi Nagaraja for the purpose of a case management inspection. LPA observed ten children supervised by three staff. LPA discussed an incident report submitted to Community Care Licensing on January 23, 2026 involving child 1 (c1). Interviews were conducted with facility staff and the parent of C1 regarding the incident. Classroom observations were made. Consistent statements were received indicating that C1 left the facility building without staff's permission and/or immediate knowledge. This incident posed an immediate risk to the health and safety of children in care. LPA Williams informed Facility Representative, Malathi Nagaraja that this report dated 2/17/2026 documents 1 Type-A citation which shall be posted for 30 consecutive days as there was immediate risk to the health, safety, or personal rights of children in care. LPA Williams informed the Facility Representative to provide a copy of this licensing report dated 2/17/2026 that documents any Type - A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with Facility Representative, Malathi Nagaraja. A notice of site visit was given to Facility Representative, Malathi Nagaraja and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
