It was also alleged that staff do not provide adequate supervision to day care children. Interviews with director, staff, and children revealed that children were never left alone at the facility. The children were supervised in the classroom, in the playground, and when they were in the restroom. Parents that were interviewed did not has any concern regarding supervision at the facility. During the visit at the facility, LPA observed that teachers were spread out and placed them self at different spots in the classroom or in the playground where the children could be supervised from different angles. LPA also observed teachers were responsive to the children and provided appropriate interactions. The facility was also alleged that staff do not ensure facility is clean, especially the children\342\200\231s restroom. Interviews with director, and staff revealed that the children\342\200\231s restroom were cleaned daily by the church\342\200\231s janitor and by the teacher as needed. It is also reveled that if more cleaning or more supplies were needed during the day, the church management could be reached anytime, and it would be solved right away. During the visit at the facility, LPA observed that the children\342\200\231s restrooms were properly maintained and cleaned. Interview with some parents revealed that they did not has any concern regarding the sanitary of the facility. It was also alleged that the staff do not ensure drinking water is accessible to day care children. Interviews with director, staff, and children revealed that children have the access for drinking water throughout the day. The children could independently pour the water from the water dispenser or request help from the teacher. During the visit, LPA observed a teacher assisted a child with the drinking water. Parents interview also revealed that children had access for drinking water throughout the day and did not have any concern regarding this. The facility was also alleged that staff does not maintain required first aid supplies. Interviews with director, staff, and children revealed that the first aid kit box were in the red backpack and was always ready and available to be used. The interview was also revealed that the first aid kits were properly maintained by the director monthly to ensure that it was not run out and always have all needed. Interview with some parents revealed that the classroom has first aid kits in the red backpack and was always ready to be used. Report Continue on 809-C The facility was also alleged that staff do not ensure outdoor play areas are free from hazards as it was not safe and cleaned. Interview with director and staff revealed that the playground was safe and appropriate for the children. It also revealed that the toys on the playground were age appropriate, the playground was fenced, all hazard items were either locked away or out of the children reach. Interview with children revealed that they were happy and safe at the facility. Parents\342\200\231 interviews revealed that the playground is safe for children and they did not has any concern with its safety. Moreover, during the visit, LPA observed that all hazard items were locked away and were not available in the playground, the toys were age appropriate, the playground was fenced and has wood chip under the climbing structure. Lastly, the facility was alleged that staff do not ensure required ratios are maintained. Interview with Director and staff revealed that there were always at least 3 teachers in the classroom or in the playground with 20 children, though most of the time it was 4 teachers with less than 20 children. The interview also revealed that 1 teacher would take a group of eight children to the restroom. The interview revealed that the facility was never out of ratio. Interview with parents revealed that they always see three or four teachers in the classroom. The interview also revealed that they did not has any concern regarding ratio capacity in the classrooms. LPA observation during the visit showed that there were always at least three teachers supervised 18 or 19 children in the classroom or on the playground. Based on the information obtained throughout the course of this investigation the above allegations, LPA Tjhia determined that the allegations were found to be UNSUBSTANTIATED, meaning although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur. Exit interview was conducted. A notice of site visit was provided and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
