Follow-up on corrections·February 12, 2026

On February 12, 2026 Licensing Program Analysts (LPAs) Deborah Khashe and Jennie Tedlos met with Facility Representative Vanaja Martha for the purpose of an unannounced case management inspection. LPAs observed 11 children being supervised by 2 staff. During the facility inspection, LPAs conducted file reviews. The Director on record with Licensing has not been employed at the facility for a few years. The facility did not notify or send a Director's Packet to licensing for the new Director, Vanaja Martha. One Title-22 Type B deficiency has been cited on a subsequent page, LIC 809-D. An exit interview was conducted, and a Plan of Correction was reviewed and developed with the Facility Representative. A copy of this report and appeal rights were discussed and left with Facility Representative Vanaja Martha. A notice of Site Visit was posted by LPA's and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty or $100.
