What the inspector wrote

Licensing Program Analysts (LPAs) David Nguyen and Corina Beckby conducted an announced field visit to the facility. LPAs toured the facility and observed eight (8) children being supervised by licensee and licensee’s assistant provider. LPAs toured the on-limits areas and pool areas. LPAs observed poisons and chemicals in the backyard and front yard accessible to children during today’s visit. In addition, LPAs observed a metal ladder propping against the pool fence. LPAs observed no body hook, pool safety daily checklist. LPAs observed a pool alarm in the original box, not in the pool water. Licensee’s spouse stated that they did not have the pool daily safety checklists and ready-to-use pool alarm. LPAs observed a gate swing away from the pool, but did not self-closed and self-latching. Based on observations and interview with licensee’s spouse, licensee did not keep the poisons and chemicals locked, and licensee did not have pool daily safety checklists, body hook, non workable self-closed and self-latched gate, and ready-to-use pool alarm, and a ladder propping against a pool fence, which poses an immediate health, safety or personal rights to persons in care. During today’s visit, LPA Nguyen conducted a face-to-face interview with licensee regarding the licensee’s absence for the day she gave birth away from her FCCH facility. Licensee stated that she had her FCCH facility opened, and she had her assistant providing care to daycare children while she was away from her FCCH facility for four (4) days. Based on interview with licensee, licensee was not present for 80% of operating hours, which poses a potential health, safety or personal rights risk to persons in care. Report continues on LIC809-C...(Page 2) (Page 2) Two (2) Type-A and one (1) Type-B deficiencies were cited on a subsequent 809-D page. Licensee was informed that this report dated 9/9/2025, documents one (1) Type-B citation and two (2) Type-A citations related to pool safety and poisons and chemicals which shall be posted for 30 consecutive days. The licensee shall also provide a copy of this licensing report to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgment of Receipt of Licensing Report (LIC9224), or other written statement, must be placed in the child’s file for verification. An exit interview was conducted, and the report was reviewed with licensee, Beenish Raheel. Licensee’s Appeal Rights were provided to licensee. A Notice of Site visit was posted by LPA and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
