Licensing Program Analysts (LPAs) David Nguyen and Corina Beckby conducted an announced field visit to the facility. As LPAs approached the front door, licensee and licensee’s spouse arrived home from their appointment. Inside, licensee’s adult daughter was caring for two (2) children. LPAs determined through accessing Guardian that licensee’s adult daughter was not cleared and/or associated to the facility. Licensee stated that she did not obtain her daughter’s fingerprint clearance. Based on observation, record review, and interview, Licensee did not ensure a criminal record clearance and/ or association of her adult daughter to her facility, which poses an immediate health, safety or personal rights risk to persons in care. During today's inspection, LPAs toured on-limits areas. LPAs reviewed children’s records and observed the Acknowledgment of Receipt of Licensing Report—LIC9224, not signed by parents in children’s files. Licensee stated that she did not provide LIC9224—Acknowledgment of Receipt of Licensing Report—to children’s parents. In the areas that were evaluated today, one (1) Type-A deficiency and one (1) Type-B deficiency were assessed on a subsequent 809-D page. An exit interview was conducted, and the report was reviewed with licensee, Beenish Raheel. Licensee Appeal Rights were provided to licensee. A Notice of Site Visit was posted by LPA and must remain posted for 30 days. Licensee’s signature on this form acknowledged the receipt of the form. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
