What the inspector wrote

Licensing Program Analysts (LPAs) David Nguyen and Corina Beckby met licensee, Beenish Raheel to conduct a proof-correction inspection to verify correction of one (1) Type-A deficiency and one (1) Type-B deficiency cited on September 15 th , 2025. There were three (3) children present during today’s proof-of-correction inspection. Licensee’s spouse was also present at the start of the inspection. On September 15 th , 2025, the facility was cited for one (1) Type-A deficiency for an individual subject to a criminal record review was not fingerprinted. During the inspection on September 15 th , 2025, licensee did not obtain her adult daughter’s fingerprint clearance. During today's inspection, LPAs toured on-limits areas. LPAs reviewed and determined through accessing Guardian that licensee’s adult daughter was cleared and/or associated with the facility. The Type-A deficiency cited on September 15 th , 2025, for an individual subject to a criminal record review was not fingerprinted, was cleared with today’s visit. In addition, on September 15 th , 2025, the facility was cited for one (1) Type-B deficiency for not having LIC9224s—Acknowledgment of Receipt of Licensing Reports—signed by parents and retaining the signed LIC9224s in children’s files. During the inspection on September 15 th , 2025, licensee stated she did not provide LIC9224s—Acknowledgement of Receipt of Licensing Reports—to children’s parents. During today’s inspection, LPAs reviewed children’s records and observed LIC9224s—Acknowledgment of Receipt of Licensing Reports—signed by parents in children’s files. Report continues on LIC809-C...(Page 2) (Page 2) The Type-B deficiency cited on September 15 th , 2025, not having LIC9224s—Acknowledgment of Receipt of Licensing Reports—signed by parents and retaining the signed LIC9224s in children’s files, was cleared with today’s visit. Proof-of-Correction Letters, Appeal rights, and a Notice of Site Visit were provided. An exit interview was conducted with licensee, Beenish Raheel. A Notice of Site Visit was posted by LPA and must remain posted for 30 days. Licensee’s signature on this form acknowledged the receipt of the form. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
