What the inspector wrote

(Page 2) In addition, information from parent’s interview disclosed that daycare child (C1) came home with soiled diaper while licensee was absent from the daycare facility due to labor and delivery, and licensee’s assistant provider provided care to daycare children. Based on interviews with licensee and reporting party and records review, LPA Nguyen was able to obtain collaborating information that licensee did not meet daycare child’s diapering needs while daycare child was in her care. Therefore, the preponderance of evidence standard has been met, and the above allegation is found to be SUBSTANTIATED . Title 22 regulations are being cited on the attached 9099-D page. An exit interview was conducted with the licensee, Beenish Raheel. A notice of site visit was provided and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Appeal rights were provided to Licensee. LPA Nguyen informed Licensee, Beenish Raheel that this report dated October 9th, 2025, documents one (1) Type-A citation. Type-A citation which shall be posted for 30 consecutive days as there is an immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Nguyen informed the licensee to provide a copy of this licensing report dated October 9 th , 2025, that documents any Type-A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. (Page 2) In addition, during the inspections and interviews with licensee, licensee’s spouse, licensee’s assistant provider, parents, and children, it was revealed that licensee provided foods to daycare children while they attended the daycare, and parents provided foods for their children when they went to the daycare. Furthermore, during the inspections and interviews with licensee, licensee’s spouse, licensee’s assistant provider, and parents, it was revealed that licensee provided close supervision by paying attention to the children or keeping their eyes on the children while the children were attending the daycare. Moreover, during the inspection and interviews with licensee, licensee’s spouse, licensee’s assistant provider, and parents, it was revealed that licensee used direction and redirection and time out with daycare children when they were in her care. Based on conflicting interviews, LPA Nguyen is unable to determine whether licensee did or did not ensure that daycare child was being adequately fed as well as that daycare children were provided with adequate care and supervision. In addition, based on observations and conflicting interviews, LPA is unable to determine whether licensee yelled at daycare child while they attended the daycare. Based on observations and the statements from licensee, licensee’s spouse, licensee’s assistant provider, parents and children and the conflicting information from reporting party, LPA determined that the complaint allegations were found to be UNSUBSTANTIATED, meaning although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur at the facility. Copy of this report was reviewed and provided to the licensee. Notice of site visit is posted and shall remain posted for next 30 days. An exit interview was conducted with the licensee, Beenish Raheel. A notice of site visit was provided and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

This poses an immediate health, safety, or personal rights risk to children in care.
