What the inspector wrote

At time 9:40am on date 04/09/2025 Licensing Program Analysts (LPAs) Janie Davis and Lauren Scott arrived at the facility to conduct an unannounced Plan of Correction (POC) Inspection. During an Annual Random Inspection conducted on date 02/25/2025, Licensee was issued a deficiency for lack of children's physician reports in file, as well as teacher qualifications. During the conversation with the Director, Aida Lomeli, agreed to adjust staffing to adequately cover ratios based on teacher qualifications, as well as obtain physicians reports and provide LPA with proof. The plan of correction due date was agreed upon for date 03/26/2025. During inspection, LPA was provided documentation of children's physician reports, and deficiency was cleared at time of inspection. However, LPA observed two (2) staff supervising 6 infants in the infant classroom. Upon record review, licensee was unable to provide proof of teacher qualifications for 2/2 staff, showing there was not a fully qualified staff caring for infants at the time. Facility did not correct the previously cited deficien cy, w hich results in the amount of $1,400.00 Civil Penalty. An exit interview was conducted with licensee. A copy of this report was read and provided. Appeal Rights were issued. Notice of Site was posted and must remained posted for 30 days.

    Cribs shall be free from all loose articles and objects, including blankets and pillows.This requirement was not met as evidenced: At time of arrival, LPAs observed one infant sleeping in a crib with a loose blanket.This poses an immediate health, safety or personal rights risk to persons in care.
