What the inspector wrote

On July 9, 2025, Licensing Program Analyst (LPA) Stacey Williams met with Director, Melanie Sandoval- Flores for the purpose of a case management inspection. LPA observed eighty eight children supervised by 8 staff ( 6 classrooms). Community Care Licensing received an incident report on June 26, 2025 indicating that a child (C1) was seen on the center's video surveillance being grabbed by their arm from staff #1 (S1) in a classroom. During today's facility inspection, LPA learned that on the day of the incident, C1's parent viewed the facility surveillance with an Administrative staff present in the front office. C1's parent notified the Administrative staff of what was being witnessed on the surveillance footage and immediately went to the classroom to discuss with S1 what she witnessed and removed C1 from the classroom. LPA reviewed approximately twenty minutes of surveillance footage during today's inspection from the incident. LPA observed S1 pointing their finger in C1's face multiple times, pushing C1 to an area of the classroom and aggressively grabbing C1 by the arm to redirect and reprimand them. This was seen more than four times and is an immediate risk to the health and safety of children in care. Director stated that she was not at the center during the time of the incident and that S1 has been placed on administrative leave. Based on observation, C1's personal rights were violated in the facility by S1. Continued on LIC 809C... Title 22 Deficiencies have been cited on the attached page, LIC 809D. Upon receipt of Type A citations, facility shall post and provide copies of the LIC 809D for parents/guardians of children currently in care and for parents/guardians of newly enrolled children for the next 12 months. Facility must also keep the signed LIC 9224, Acknowledging Receipt of Licensing Report LIC 809D in each child's files. Exit interview conducted at which time the report was reviewed with Director, Melanie Sandoval- Flores. A Notice of Site Visit was posted by LPA Williams and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.00.
