What the inspector wrote

On July 23, 2025, Licensing Program Analyst (LPA) Stacey Williams met with Director, Melanie Sandoval-Flores for the purpose of case management inspection. LPA observed (93) ninety three children supervised by 9 staff. Community Care Licensing received an incident report on May 1, 2025 stating that on April 30, 2025, during classroom pickup time, child (C1) was seen running to their parent crying. Interviews were conducted with C1’s parent and staff present in the classroom regarding the incident. C1’s speech is limited; however, it was reported that C1 repeated the words “boo-boo back” which prompted C1’s parent to lift their shirt to see their back. Several red marks resembling fingerprints were noticed on C1’s back. Staff present in the classroom were unable to determine how the injury occurred. Based on the information received, the injury is a result of a lack of supervision in the classroom. Title 22 regulations cited on subsequent page, LIC809D. Upon receipt of Type A citations, facility shall post and provide copies of the LIC 809D for parents/guardians of children currently in care and for parents/guardians of newly enrolled children for the next 12 months. Facility must also keep the signed LIC 9224, Acknowledging Receipt of Licensing Reports LIC 809D, in each child's files. Exit interview conducted at which time the report was reviewed with Director, Melanie Sandoval- Flores. A Notice of Site Visit was posted by LPA Williams and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.00.

    The licensee shall comply with all terms and conditions set forth in the admission agreement.This requirement was not met as evidenced by: Center Management staff terminated enrollment of C1 which is not in accordance with the parent handbook. This is a potential risk the health and safety of children in care.

    The licensee shall provide care and supervision as necessary to meet the children's needs. This requirement was not met as evidenced by: C1 sustained an injury on their back that staff was unable to determine how the injury occurred. The injury resembled fingerprint marks. This is an immediate risk to the health and safety of children in care.
