What the inspector wrote

On 08/13/2025, Licensing Program Analyst (LPA) Corina Beckby conducted a Case Management visit for the purpose of increasing the licensed capacity from 8 to 14 children. LPA met with Licensee, Kalyani Gajavada. Licensee was present supervising 4 daycare children including 3 infants (ages 2 yrs old, and three 22m olds). Facility hours of operation are Monday through Friday 9:00 am to 5:30 pm. Licensee applied to increase her capacity from a Small Family Child Care Home to a Large Family Child Care Home. Fire clearance was granted by South County Fire Authority Community Risk Reduction on 08/06/2025. LPA verified the annual fees are current. All individuals subject to criminal background review have obtained a criminal record clearance. A health and safety inspection was conducted in all areas accessible to children. Home appears orderly and suitable for children. OFF-limit areas are the garage, 2 nd floor, walk in pantry, 2 storage closets, right and left side of backyard and front yard. Hazardous items were stored inaccessible to children. Napping equipment and age-appropriate toys were observed. Licensee stated there are no weapons in the home. Home has a working telephone, a 3A40BC fire extinguisher, and dual functioning smoke and carbon monoxide detector. The backyard is fenced for supervision. Licensee understands that prior to making alterations or additions to the home or grounds, the licensee shall notify the Department of the proposed changes. Current CPR & First Aid was verified and expires on 09/07/2026 . Mandated Reporter expires on 02/20/2026 . LPA reviewed large family childcare home capacity limitations with licensee. Continued on LIC 809-C... page 2... Capacity increase is approved as of today, 08/13/2025. The facility is licensed to serve a MAX. CAP (WHEN THERE IS AN ASSISTANT PRESENT): 12 - NO MORE THAN 4 INFANTS. CAP 14 - NO MORE THAN 3 INFANTS. 1 CHILD IN KINDERGARTEN OR ELEMENTARY SCHOOL AND 1 CHILD AT LEAST AGE 6. LICENSEE UNDERSTANDS THAT WHEN ASSISTANT IS NOT PRESENT, THE CAPACITY REVERTS TO A SMALL FAMILY CHILD CARE HOME. LPA informed Licensee, Kalyani Gajavada, that this report dated 08/13/2025, documents a Type B citation that is a potential Health and Safety, or Personal Rights risk to persons in care. A separate 809D is issued for the deficiency. An Exit interview was conducted, and the report was reviewed with Licensee, Kalyani Gajavada. LPA posted a notice of site visit. Licensee understands the Notice must remain posted for 30 days and that a failure to comply with posting requirements shall result in an immediate civil penalty of $100. Appeal Rights were provided. A copy of this report will remain on file for a period of three years for public review upon request. The Licensee's signature on this form acknowledges receipt of this form.
