What the inspector wrote

On 04/10/25, Licensing Program Analyst (LPA),Elvira Sierra conducted an unannounced complaint inspection visit and met with the facility Director, Tyanne Woodyard Smith. Upon arrival present in the facility LPA observed 4 staff supervising 28 napping children. LPA inspected the facility in all areas accessible to children for health and safety. LPA also interviewed staff. LPA observed dirty diapers in the garbage can in the intro to preschool classroom and two carpets that had dirt stains in need of cleaning and vacuuming. LPA explained to the Director that floors shall be swept and mopped with a disinfecting solution at leas daily or more often if necessary. Based on LPA’s observations the preponderance of evidence standard has been met, therefore, the above allegation is found to be SUBSTANTIATED. Deficiency cited on the attached LIC 9099-D. An exit interview was conducted in which the report and Appeal of Rights were reviewed and provided to the Director, Tyanne Woodyard Smith.

    101638.1 General Sanitatione) Washing, cleaning and sanitizing requirements for areas used by staff and children, or for areas that children have access to, shall include:(1) Floors, except those carpeted, shall be vacuumed or swept and mopped with a disinfecting solution at least daily, or more often if necessary. (2) Carpeted floors and large throw rugs that cannot be washed shall be vacuumed at least daily and cleaned at least every six months, or more often if necessary. This requirements was not met as evidence by: LPA observed dirty diapers in a open container in the intro to preschool classroom and carpets that had dirt and stains. Floors also in the classroom need to be cleaned. This is a deficiency that if not corrected can pose a risk to health to the children in care.
