On 02/13/25 Licensing Program Analysts(LPAs) Elvira Sierra, Sarah Tibbett and Licensing Program Manager, Chayntel Hunter met with Director, Tyanne Woodyard Smith to follow up on an Unusual Incident Report (UIR) occurred on 02/11/26. During today's visit LPAs and LPM inspected the facility and observed the care and supervision of children. Present in the facility were 7 infants and 29 preschool children supervised by 9 staff. Also present were three Behavior Specialist. The facility reported the UIR to Community Care Licensing within 24hrs. A written UIR was submitted within 7 days, describing the specifics of the incident. Facility evaluation report was reviewed and discussed with Director, Tyanne Woodyard Smith. Exit interview was conducted. A Notice of Site Visit posted and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. In the areas that were evaluated, no deficiencies were cited during today's inspection.
