On February 23 rd , 2026, at 11:00 AM, Licensing Program Analyst (LPA) David Nguyen conducted an unannounced field visit to the facility and met with facility representative/director, Tyanne Woodyard Smith to follow up on an Unusual Incident Report (UIR) occurred on February 11 th , 2026. During today's visit, LPA inspected the facility and observed the care and supervision of children. Present in the facility were eight (8) infants and twenty-eight (28) preschool children supervised by seven (7) childcare staff. Also present were three (3) Behavior Specialists. During the inspection on February 13 th , 2026, licensing staff conducted onsite face-to-face interviews with facility representative/director and childcare teachers to follow up with the Unusual Incident Report (UIR) reported to California Department of Social Services (CDSS), Community Care Licensing Division (CCLD) on February 11 th , 2026, via telephone and submitted the UIR to CDSS, CCLD on February 12 th , 2026, via email. During the interviews, licensing staff learned that on February 11 th , 2026, Staff 1 (S1) was observed to be intoxicated while caring for children. Childcare staff who observed the incident reported this to the Director Tyanne Woodyard Smith, who called the police to have S1 removed. S1 resigned the same day. In addition, licensing staff learned that S1 supervised childcare children for approximately thirty (30) minutes but was never alone with the childcare children. Report Continues on LIC8098-C... (Page 2) (Page 2) During today’s visit, LPA informed facility representative/director Tyanne Woodyard Smith that this report dated 2/23/2026, documents one (1) type-A citation. Type-A citation which shall be posted for 30 consecutive days as there are immediate risks to the health, safety, or personal rights of children in care. Also, LPA David Nguyen informed the facility representative/director to provide a copy of this licensing report dated 2/23/2026 that documents any Type-A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement or Receipt of Licensing Report (LIC9224) or other written statement, must be in the child’s file for verification. LIC9224 and Appeal Rights were provided. An exit interview was conducted, and the report was reviewed with facility representative/director, Tyanne Woodyard Smith. A Notice of Site visit was posted by LPA and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Facility representative/director, Tyanne Woodyard Smith acknowledges, that for TYPE-A DEFICIENCIES ONLY upon receipt, director shall post LIC 809-D.
