What the inspector wrote

On May 21 st , 2026, Licensing Program Analysts (LPAs) David Nguyen and Deborah Khashe met with Facility Representative/Director, Tyanne Woodyard Smith to follow up on an Unusual Incident Report (UIR) occurred on May 19 th , 2026. During today's visit, both LPAs Nguyen and Khashe inspected the facility and observed the care and supervision of children. Present in the facility were nine (9) infants and twenty-two (22) preschool children supervised by six (6) childcare staff. The facility self-reported the Unusual Incident Report (UIR) to the Community Care Licensing Division within 24hrs. A written UIR was submitted within 7 days, describing the specifics of the incident. Facility evaluation report was reviewed and discussed with Facility Representative/Director, Tyanne Woodyard Smith. Exit interview was conducted. A Notice of Site Visit posted and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
