Centralized Application Bureau (CAB)
Licensing Program Analyst (LPA) Elvira Sierra and Erwin Tjhia met with Facility Representative (FR) Nicole Nino for the purpose of an announced Prelicensing inspection. Applicant is requesting to add preschool component of 20 preschool children 2-6 years old to current license. The facility is currently licensed for infant component for 8 infants ages 12-24 months and 16 toddlers ages 18-36 months under toddler option program. FR understands that any children over 5 years old can attend the program if they are not enrolled in a TK, Kinder or elementary grade. The program will operates inside the Grace Community Church, Monday through Friday from 07:30 a.m. to 05:30 p.m. in classrooms #1 (infant) #2 and #3 (Toddlers), and # 4 (preschool). The Department received a granted fire clearance on 07/02/25.
The facility will be providing breakfast, lunch, snacks. Meals are prepared by an outside vendor and deliver to the facility every Tuesday and Friday.
Facility is currently closed for preservices and no daycare children were present. LPAs informed FR that the facility will be converted to a single childcare center license under facility number 394501601, therefore LPA Sierra will be closing the current infant/toddler license under facility number 394500717.
LPAs reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child\342\200\231s records, personnel records, administrative records, and documents to be posted.

LPAs discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at

as an additional resource. LPAs also informed of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at:

, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
INDOOR ACTIVITY SPACE:

preschool classroom located in Room 4. LPAs observed a sufficient amount equipment, toys, tables, chairs, and cubbies for the daycare children.
FR acknowledges there will be no infants under 12 months old due to no available cribs. There is a first aid kit in each of the classrooms. Medications will be stored in the locked classroom cabinets. LPAs observed napping cots for infants and preschool children. LPAs observed cleaning disinfectants are appropriately stored and inaccessible to children. There are a total of 7 toilets and 4 sinks in the children bathrooms and two permanent and 4 portables sinks available in the classrooms. Staff uses a separate bathroom. FR stated there are no poisons or firearms on the premises.

stated there will be a water jug and cups available in the classrooms. LPAs observed a functional dual carbon monoxide/smoke detectors in the classrooms. LPAs observed an electronic sign-in/sign-out system.
LPAs measured the preschool classroom (Room # 4). Infant and toddler classrooms were previously measured and FR stated that no additions or changes have been made to the infant and toddlers\342\200\231 classrooms. LPAs used measurements from report dated 11/14/22 under the infant/toddler license

, therefore LPAs did not take measurements for the infant/toddler classrooms. The preschool classroom space (Room # 4 contains a total of 652 square feet which will accommodate 18 preschool children. The infant classroom (Room # 1) and the two toddler classrooms (room #2 and # 3) contains a total of 972 square feet, which will accommodate the request capacity for 8 infants and 16 toddlers.

OUTDOOR ACTIVITY SPACE
There are two yards on the property, Infant Yard, and Toddler Yard. The outdoor play areas are fenced with a
chain link
fence that is at least four feet tall.

in the toddler or infants play yards.
LPAs observed a sufficient amount of equipment and toys. There are no bodies of water on the premises. There are shaded areas supplied by a tree in the toddler play yard and a canopy in the infant play yard. LPAs measured outdoor area during today\342\200\231s visit. The infant outdoor area contains a total of 1514.34 which can accommodate up to 20 infants. The toddler outdoor area contains a total of 1701.2 which will accommodate up to 22 toddlers at one time

Facility requested a rotational outdoor scheduled time waiver to shared toddler yard
with preschool students.

LPAs discussed the following: 100% supervision is required at all times; personal rights; inspection authority; reporting requirements; staff to children ratios and capacity; staff qualifications; and maintaining buildings and grounds. LPAs discussed with FR any changes that may occur regarding the director or an employee acting in the director's absence must be reported to department within 10 working days.
Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

FR was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
Written Directives for Lead Testing in Child Care Centers: Assembly Bill 2370, Chapter 676, Statutes of 2018, added Health and Safety Code section 1597.16 requiring all licensed Child Care Centers constructed before January 1, 2010, test their water for lead between January 1, 2020 and January 1, 2023, and then every 5 years after the date of the first lead testing. For a license issued on or after July 1, 2022, initial testing results shall be received and posted within 180 days of licensure.
