What the inspector wrote

On 5/21/26 Licensing Program Analyst (LPA) Sarah Tibbett met with Applicant, Rabeeha Khan, for the purpose of conducting a change of location inspection. Applicant is requesting a change of location from previous facility #394500403 to current location. Licensee’s assistant and minor child were also present during the inspection. Applicant's tentative operating hours are Monday-Sunday, up to 24 hrs a day. LPA discussed annual fees and provided an Entrance Checklist. The facility is a two story home that consists of 3 bedrooms, and 3 bathrooms. LPA and applicant toured the entire home inside and outside. Off limit areas include the entire upstairs, laundry, and garage . Fire extinguishers located in the living room and laundry. Dual smoke and carbon monoxide detectors were observed to be in operational order. LPA discussed the requirement of the fire drill log. Licensee understands that they must conduct fire drills as well as log the fire drills at least once every six (6) months. Licensee stated there are no weapons in the home. There are no bodies of water on the property. There is a fireplace in the upstairs area that is screened. Hazardous items are made inaccessible to children. Licensee has current mandated reporter training and current pediatric CPR and first aid training. Report continues on 809-C... Licensee acknowledged that children are never allowed in the off limit areas. Off limit areas will remain inaccessible by closed doors and baby gates. Licensee understands that if any structural changes are made to the home; licensing must be notified prior to construction. Licensee understands that if they want to make any off-limit area an ON-limits area, they must notify licensing and LPA must do an inspection BEFORE children are allowed in the area. Backyard is completely fenced. Licensee understands that 100% supervision is required when children play in any unfenced areas. LPA discussed the safe sleep regulations with Licensee and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep , as an additional resource. LPA also informed Licensee of the importance of checking for and removing recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Licensee understands that baby walkers and bouncers are not allowed. LPA discussed the requirement to check and log infant napping every 15 minutes for infants under 24 months. LPA also discussed the LIC 9227, Individual Sleeping Plan, to be completed for infants under 12 months. Applicant currently does not have any children enrolled that require IMS. LPA discussed IMS services and the requirement to create a plan of operation. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Report continues on 809-C... Licensee understands that anyone living or working in the home, eighteen years of age or older must obtain fingerprint clearance PRIOR to living or working in the home. Licensee understands that licenses are not transferable, and once licensed, licensee must live in the home and be present for 80% of the operating hours. Licensee understands that if an unusual incident occurs; licensing is to be notified via phone call, e-mail or fax within 24 hours and the Unusual Incident Report LIC 624B shall be submitted within 7 days to remain in compliance. Licensee understands that children’s records are to be maintained according to Title 22 regulations and be accessible to licensing for up to three years. On this date, 04/24/2026, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility address. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ. This facility evaluation report was reviewed and discussed with the Applicants. Records, postings and reporting requirements were discussed. Licensee was encouraged to visit the department website at WWW.CCLD.CA.GOV fo r information regarding child care updates, forms, regulations and legislation pertaining to family child care homes. LPA discussed the capacity regulations with licensee. Licensee understands that their own children under the age of 10 count as part of the ratio. Effective today, 5/21/26, facility is approved for a Large Family Child Care License to serve 12 children (when there is an assistant present) with no more than 4 infants, or capacity of 14 children when 1 child is enrolled in Transitional Kindergarten or above and 1 child at least age 6 with a maximum of 3 infants. Without assistant, the ratios revert to those for small family childcare home.
