What the inspector wrote

On 12/15/2025 at 3:31 PM Licensing Program Analysts (LPAs) Bill Billones and Elizabeth George conducted an unannounced Case Management Deficiencies Inspection at the abovementioned Child Care Center (CCC). Upon arrival, LPAs met with Site Director Bonnie Luther and informed them of the purpose of the inspection. LPA confirmed hours of operation are from 7:30 AM to 4:30 PM, Monday through Friday. LPAs noted 16 children in care with 3 staff providing care and supervision. LPAs were escorted throughout the facility. During the inspection, LPAs observed 1 child (C1) unsupervised and alone in the restroom of the Red Door classroom with the door closed. Lead teacher (S1) of the classroom acknowledged and informed LPAs that C1 was using the restroom during the LPAs head count. LPAs advised Site Director this is not in compliance with Title 22 regulations and a deficiency will be cited. In addition, LPAs review of the Guardian facility\342\200\231s roster revealed one staff member (S2) was not associated to the center. Site Director provided an updated LIC9182 and LPAs transferred S2 (who has an active Guardian clearance) to the center during today's visit. A technical violation will be issued. Based on the information gathered during today\342\200\231s inspection, 1 Type B deficiency was cited under Title 22 Regulations, which can be found on the attached 809-D. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Exit interview conducted, appeal rights were provided, and report was reviewed with Site Director Bonnie Luther.

This requirement was not met as evidenced by: Based on observation, the Licensee did not comply with the section cited above by 1 children left unsupervised in the classroom's restroom, which posed a potential health, safety, and personal rights risks to children in care.
