Inspector’s narrative

What the inspector wrote

According to the Complainant, on 7/2/25 around approximately 5:00 PM, they arrived at the facility and were informed that their child had already been picked up by the child's mother. Upon entering the premises, the Complainant found their child was left unattended near the playground. The child appeared to have been left outside for an extended period of time and was also observed to have soiled clothing. The Complainant also observed and reported a number of other children left inside of a classroom lacking teacher supervision. Based on interviews conducted and documentation reviewed, LPA determined that a child in care was left unsupervised in soiled clothing for approximately thirty minutes, and that other children were also briefly left without supervision. These occurrences constitute violations of personal rights and care and supervision requirements. Therefore, the preponderance of evidence standard has been met, and the above allegations are found SUBSTANTIATED. Two Type A deficiencies are being cited and an Immediate Civil Penalty is being assessed per Health and Safety Code and Title 22 regulations (see LIC 9099-D). Upon receipt, the Director shall provide copies of this licensing report to parents/guardians of children in care and to parents/guardians of children newly enrolled at the facility during the next 12 months. The Acknowledgement of Receipt (LIC 9224) must be given to parents and shall be completed and signed by each parent/guardian with copies maintained in each child's file. Director was given a copy of LIC 9224 Acknowledgement of Receipt of Licensing Reports. Director was informed that any additional Type A deficiencies may result in further administrative action against the CCC. A Notice of Site Visit (LIC 9213) was given. Appeal Rights (LIC 9058) were also given and signature on this form acknowledges receipt of these rights. LPA reminded Director to ensure Notice of Site Visit form is posted or a civil penalty of $100 may apply.
