What the inspector wrote

Based on LPA's observations and interviews, the preponderance of evidence standard has been met, therefore, the above allegations are found to be SUBSTANTIATED. As a result, two Type B deficiencies are being cited today under California Code of Regulations, Title 22, Division 12 on the attached LIC9099Ds. Director was reminded that this report must be made available to the authorized representatives of children in care and maintained in the facility file for three years. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Exit interview conducted, appeal rights were provided, and report was reviewed with the Director Angelica (Gallardo) Jepsen .
