On 5/5/25, at 1:46 PM, Licensing Program Analyst (LPA) Shane Loftus conducted an unannounced Case Management inspection of the above-mentioned Child Care Center (CCC) to follow up on the report of an Unusual Incident Report (UIR) received by the Department on 5/1/25. Specifically, a child in care, C1, sustained an injury to their left eye area. The injury occurred after C1 went down the slide in the toddler classroom. C1 lost his balance while getting up and hit his left eye area on the corner of the adjacent sink. LPA met with CCC Director Ruby Alcantar and explained the nature of the inspection. LPA notes 9 children are on site along with 3 teachers providing care and supervision. LPA and Director viewed area where C1 injured his left eye area. LPA notes the classroom has carpet to absorb the children’s fall at the base of the slide. Director informed LPA the activity C1 was participating in was a normal activity and there was no inherent hazard. Director cleaned the injury area and applied an ice pack to reduce the swelling. Director informed the parents of the child immediately and provided pictures of the injury. LPA viewed the pictures and notes there was no bleeding. To date, C1 is currently attending the CCC and has no prolonged issues related to the injury. Based on observations and the interview with the Director, it is determined neither the equipment at the CCC nor the actions of the CCC's staff contributed to C1's incident. No deficiencies were cited during today’s inspection. LPA provided a Notice of Site Visit which is to be posted as required by H&S Code Sec.1596.817. The Notice of Site Visit must remain posted for 30 days or a civil penalty $100.00 may apply.
