Inspector’s narrative

What the inspector wrote

On January 7, 2026 at 12:31 PM, Licensing Program Analysts, Gigi Reyes and Matthew Sapien, conducted an unannounced annual inspection at the above Child Care Center (CCC). LPAs met with Director Rhiana Merigold and explained the purpose of the inspection. CCC utilizes classrooms 1701 and 1702 at Santa Rosa Academic Academy. The CCC operates Monday\342\200\223Friday, 7:00 AM\342\200\2236:00 PM. LPAs observed seventeen (17) Transitional Kindergarten students being escorted by Teacher Caleb Biel from the Elementary School to the KCE Champions school-age area, where they remained under the supervision of two (2) staff members. It was also noted that Area Manager Dawn Martin was present during the inspection. LPAs observed the licensing requirement posted in the classroom. Updated snack menu was also posted. Classrooms were clean, organized, equipped with age-appropriate toys, books, and equipment. The CCC provides morning and afternoon snacks; Drinking water is available via a dispenser, and children also bring bottled water from home . Sign-in/out sheets reflected children\342\200\231s arrival and departure times, verified with authorized signatures . The CCC restrooms are located, outside the classrooms. All restrooms were clean and in good working condition. The CCC shares these facilities with other students; however, children are escorted to the restroom by CCC staff, and when necessary, the entire class goes together to ensure proper supervision. The outdoor play area is fully enclosed with chain-link fencing, and gates are secure. Equipment is age-appropriate, and the play structure has sufficient cushioning material (wood bark) to absorb falls. Children also have access to basketball courts and blacktop areas. Shade is provided by building overhangs. No bodies of water were observed on site. CCC Director confirmed no firearms or ammunition are stored on site. Staff and children\342\200\231s records were reviewed. Children\342\200\231s files included emergency contact information and admission agreements. Staff files contained current EMSA-approved Pediatric CPR and First Aid certifications, valid until 03/30/2027 , as well as Mandated Reporter training valid until 12/18/2026 . It was noted that Staff 1\342\200\231s Mandated Reporter training had expired . Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP . When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California . LPA verified that the lead testing was completed in accordance with the Written Directives outlined in PIN 21-21.1-CCP . During today\342\200\231s inspection, deficiency was cited under Health and Safety Code Notice of site visit was given and must remain posted for 30 days.
