On 1/7/26, at 10:00 AM, Licensing Program Analysts (LPAs) Matthew Sapien and Gigi Reyes made an unannounced Case Management inspection at the abovementioned Child Care Center (CCC). LPAs met with Dawn Martin, Area Manager of the CCC and explained the nature and purpose of the inspection. LPAs, in the company of the Area Manager, toured the CCC. At the time of the inspection, LPAs observed 0 children in care. In addition to the Site Supervisor of the CCC, LPAs also observed 2 additional staff member at the time of the inspection (each cleared). Importantly to note, the CCC's current hours of operation are 7:00 AM until 6:00 PM Monday through Friday. This Case Management inspection follows an Unusual Incident at the CCC which occurred on 12/29/25. On 12/31/25, it was reported to the Department via telephone call. This incident involved an inappropriate interaction between two children in care. During today's inspection, LPAs conducted an interview with three staff members, including the Area Manager about the incident. During today's inspection, one Type A citation was issued (see LIC 809-D page). Upon receipt, the Facility Representative shall provide copies of this licensing report to parents and guardians of children in care and to parents and guardians of children newly enrolled at the facility during the next 12 months. The Acknowledgement of Receipt of Licensing Reports (LIC 9224) shall be completed and signed by each parent and guardian with copies maintained in each child's file. The Area Manager was informed that any additional Type A deficiencies may result in further administrative action against the CCC. LPAs reminded the Facility Representative of the importance of direct supervision over children in care at all times. A notice of site visit was given and must remain posted for 30 days. Appeal rights were also given and signature on this form acknowledges receipt of these rights. Exit interview was conducted and report was reviewed with Area Manager, Dawn Martin.
