What the inspector wrote

On April 2, 2026 at 8:55 AM Licensing Program Analyst (LPA) Elizabeth George conducted an unannounced Case Management Inspection. LPA met with Site Supervisor Sorcorro Cano and discussed the purpose of the inspection was to follow up on an incident reported to the Department on 3/23/26 as required. LPA in the company of the site supervisor toured the facility inside and outside. During the inspection, LPA observed 5 staff providing care to 17 children. On March 23, 2026 at approximately 10:30 AM, Child was playing outside with a standing basketball hoop, the child was hanging from the top of the hoop and swung lifting feet in the air. When the child came down the child's private area was scratched due to bolts that should been under the hoop but were loose and accessible. The child did not let teachers know that they were scratched or hurt throughout during the day. The child's mother noticed there was blood in the child's underwear after arriving home and immediately returned to the center. The center staff immediately removed the basketball hoop from the facility premise and are taking measures to ensure equipment used in the outdoor play area are safe for children. Based on the information gathered during the inspection, LPA determined that the staff took appropriate action to meet the needs of the child and other children in care. No deficiencies were cited as a result of the incident. Exit interview conducted and report was reviewed with the Site Supervisor. A notice of site visit was given.
