What the inspector wrote

On 5/2/25 at 1:15 PM, Licensing Program Analyst (LPA) Matthew Sapien conducted an unannounced Case Management Inspection at the abovementioned Family Child Care Home (FCCH) to deliver a follow up Decision and Order pertaining to Accusation/Exclusion Action (CDSS No. 6424142101) in the matter of Amanda M. Alvarez. LPA met with Licensee, Heather Luis, and discussed the nature and purpose of the inspection. During the visit, LPA observed the Licensee\342\200\231s spouse (cleared and associated) and 0 children in care. Importantly to note, LPA observed a construction company working on the flooring throughout the house upon arrival. LPA informed Licensee to update the Department in future instances like this ideally. LPA explained to the Licensee the Accusation/Exclusion Action which prohibits Amanda Alvarez from employment in, presence in and from contact with clients, of any facility licensed by CDSS or certified by licensed foster family agency or resource family home for the remainder of the Respondent\342\200\231s life. The following documentation was provided and explained: \302\267 Decision and Order \342\200\223 Accusation/Exclusion Action - CDSS No. 6424142101 As aforementioned during the previous visit conducted on 3/21/25 by LPAs Gigi Reyes and Matthew Sapien, Licensee stated that sometime around 2021, Amanda Alvarez, applied for an assistant position at the FCCH, but was never fully employed by the FCCH due to said individual\342\200\231s fingerprints not clearing. In October 2024, Licensee disassociated Amanda Alvarez from her facility roster through Guardian. During today's inspection, no deficiencies were cited. (CONT. LIC 809-C, Page 2) During today's inspection, no deficiencies were cited. A Notice of Site Visit (LIC 9213) was issued and must remain posted for 30 days or a civil penalty of $100 may apply. Appeal Rights (LIC 9058) were also given to the Licensee and signature on this form acknowledges receipt of these rights. Exit interview conducted and report was reviewed with the Facility Representative, Heather Luis.
