What the inspector wrote

On 11/21/25, at 4:30 PM, Licensing Program Analysts (LPAs) Matthew Sapien and Gigi Reyes conducted a collateral inspection at the abovementioned CCC. LPAs met with Mallory Cleaver, Lead Teacher of the CCC, and explained the nature and purpose of the inspection. The LPAs, in the company of the Lead Teacher, toured the CCC. At the time of the inspection, LPAs observed 13 day care children present. Around 3:51 PM, the number of children in care reduced to 11 due to parent pick ups. Only the Lead Teacher was observed at the time of the inspection in terms of staffing. During today's inspection, LPA reviewed relevant files of the CCC and interviewed Staff Member 1 (S1) in relation to the incident that occurred at The Learning Tree, Golden Hill Road location During today's visit, no citations were issued. A notice of site visit was given and must remain posted for 30 days. Appeal rights were also given and signature on this form acknowledges receipt of these rights. Exit interview was conducted and report was reviewed with the Facility Representative, Mallory Cleaver.
