Inspector’s narrative

What the inspector wrote

On November 21, 2025, at 3:45 PM, Licensing Program Analysts (LPAs) Gigi Reyes and Sapien conducted an unannounced case management inspection at the Child Care Center. Upon arrival, the LPAs met with Teacher Mallory Cleaver and explained the purpose of the inspection. As the LPAs approached the facility, LPAs observed Child #1 exiting the center and being picked up by a parent. When the LPAs entered the classroom, Child #2 was also in the process of being picked up. LPAs asked Ms. Cleaver how many children were currently present. Ms. Cleaver responded that there were 12 children, with one being picked up. LPAs clarified that including Child #1, the total count was 13 children. LPAs then asked Ms. Cleaver if she had an assistant present. Ms. Cleaver stated that her assistant had left at approximately 3:30 PM. Based on observation and record review, between the hours of 3:31 PM to 3:45 PM, Staff 1 was supervising 13 pre school children. LPAs reviewed the sign-in/sign-out sheets, it was noted that sign in sign out does match with the number of children present on or about 3:45 PM.
