(a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance, except as specified in (b) and (c) below.This requirement is not met as evidenced by: Based on LPAs’ observation and interview with Staff #1, between 3:30 and 3:45 PM Staff #1 was supervising 13 children, which poses an immediate risk to the health and safety of children in care.”
