What the inspector wrote

On December 3, 2025 at 1:46 PM, Licensing Program Analysts (LPAs) GIgi Reyes and Matthew Sapien conducted an unannounced Case Management inspection at the above Child Care Center and met with the Director, Kelsey Sullivan. LPAs explained the purpose of the visit. Together, LPAs and the Director toured the facility. During the inspection, LPAs observed 21 children resting in the nap area, supervised by three staff members: the Director, Staff 1, and Staff 2. A review of Guardian Background Check System and Personnel list revealed that Staff 1, Kamryn L. Clarkson, does not have a Criminal Record Clearance on file. Records further indicated that Staff 1 has been working at the center since November 6, 2025. As a result of this violation, LPAs assessed a Civil Penalty of $500. LPAs informed the Director that Ms. Clarkson cannot be present in the Child Care Center until proper Criminal Record Clearance is obtained. During today's inspection, Type A deficiency was cited. LPAs informed the Director to provide a copy of this licensing report dated 12/3/2025, that documents a Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with Director, Kelsey Sullivan Notice of Site Visit was issued and must remain posted for 30 days. Appeal Rights were issued and explained. Please note that the report was created in error under facility number 406216212
