What the inspector wrote

On 12/10/25, at 11:45 AM, Licensing Program Analysts (LPAs) Matthew Sapien and Gigi Reyes conducted an unannounced plan of correction (POC) visit for the abovementioned CCC. This inspection is to follow up on a Case Management (Deficiencies) inspection conducted on 12/3/25. LPAs met with Kelsey Sullivan, Director of the CCC, and explained the nature and purpose of the inspection. The LPAs, in the company of Director, toured the CCC. At the time of the inspection, LPAs observed 27 day care children present. In addition to the Director, LPAs observed 2 Staff Assistants (Teachers; cleared and associated). During the 12/3/25 visit, LPAs issued one Type A deficiency. On that date, LPAs issued a Type A deficiency due to an individual by the name of Kamryn Clarkson working at the CCC not having Criminal Record Clearance (CRC) as required by the Department. In addition to the Type A deficiency, a Civil Penalty of $500.00 was also given. Plans of correction were also given to the facility and were listed as the following: \302\267 The Director shall remove Kamryn Clarkson from the CCC and she shall not return until CRC is obtained \302\267 A written plan of correction shall be sent to the Department detailing how the center will ensure all staff are fingerprinted and cleared before working \302\267 Parents shall be given copies of the 12/3/25 report, the report shall be posted prominently in the CCC, and parents shall return the LIC 9224 where the form will be maintained in each of the children\342\200\231s files. During today's inspection, LPAs viewed the number of children and staff present. Through interview and observation, LPAs determined that all individuals working at the facility have CRC. During today's visit, no citations were issued. A notice of site visit was given and must remain posted for 30 days. Appeal rights were also given and signature on this form acknowledges receipt of these rights. Exit interview was conducted and report was reviewed with the Facility Representative, Kelsey Sullivan.
