What the inspector wrote

Based on LPAs interview, observation, and record review, the preponderance of evidence standard has been met, therefore the above allegation is found SUBSTANTIATED. California Code of Regulations, Title 22, Division 12 or Health and Safety Code are being cited on the attached LIC 9099-D. Upon receipt, the Co-Director shall provide copies of this licensing report to parents/guardians of children in care and to parents/guardians of children newly enrolled at the facility during the next 12 months. The Acknowledgement of Receipt (LIC 9224) shall be completed and signed by each parent/guardian with copies maintained in each child's file. The Co-Director was given a copy of LIC 9224 Acknowledgement of Receipt of Licensing Reports. Co-Director was informed that any additional Type A deficiencies may result in further administrative action against the CCC. A Notice of Site Visit (LIC 9213) along with Appeal Rights (LIC 9058) were provided. LPA reminded Co-Director to ensure Notice of Site Visit form is posted prominently or a civil penalty of $100 may apply. LPAs reminded Co-Director to ensure Notice of Site Visit form is posted for thirty days.

Based on interview, observation, and record review, LPA's observed 34 children in care along with the Co-Director, 1 Teacher, and 1 Teacher's Aide. The proper staffing ratio is not being met which poses an immediate risk to health and safety of children in care.
