On September 27, 2024, at 9:10 AM, Licensing Program Analyst (LPA) Gigi Reyes and conducted an unannounced Annual Random inspection at the above Family Childcare Home (FCCH) and met with Licensee, Irma Salguero. LPA discussed the purpose of the inspection. The FCCH operates Monday to Friday, 6:00 AM to 5:30 PM and provides care to children 10 months to 11 years old. When LPA arrived, LPA observed 2 infants, Infant 1 and Infant 2 laying in the car seats covered with blankets in the living room. When asked about the infants being in the car seat, Licensee responded they were just dropped off by the parent. In the middle of the inspection, LPA asked Licensee what time the two infants were dropped off. Licensee stated, at 6:30 AM. LPA and licensee continued the tour of the home, LPA observed two infants, nIfant 3 and and Infant 4 in the crib while awake, LPA reminded licensee that infants were not supposed to be in the crib as cribs are for sleeping purposes only. LPA observed a total of nine (9) children consisting of four (4) infants and five (5) older children under the care of licensee Irma Salguero alone. At approximately 9:27 am, assistant, Mirna Leon arrived. Continued on LIC 809C Following are additional observations at the FCCH that are in violations of California Code of Regulations 1. Disaster plan LIC 610A is not posted, licensee stated she does not have it 2. Facility sketch (LIC999) is not available or not on file. 3. Fire and disaster drill log is lost, per licensee FCCH conducts fire and disaster but log was missing. 4. Assistant, Mirna Leon has no immunization record. 5. Licensee and Assistant did not renew the Mandated Reporter Training 6. It was observed that a wall was constructed in the garage to create an additional room for children to play, the modification was not permitted per licensee and was not reported to CCL prior to construction and its use. 7. The un permitted and off-limit area is being used by children to play. 8. The off-limit room at the back is being used by children for napping. 9. An individual Safe Sleep plan for infant 1 and infant 3 who were enrolled under the age of 12 months were not on file 10. The documentation for napping infants is incomplete. LPA noted that fire extinguisher was serviced on 9/1/2024, carbon monoxide and smoke detectors were tested and found functional. Licensee\342\200\231s Pediatric first aid and CPR was renewed on 5/13/2023 and expires in 2025. Bathroom for children\342\200\231s use is free of toxins. Age-appropriate toys, and equipment were observed inside the home. LPA observed that spa is covered and locked. Backyard is fully fenced and free of tripping hazards. Licensee, was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee Irma Salguero and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. Continued on LIC 809C LPA also informed licensee, Irma Salguero the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA Reyes informed licensee, Ms. Salguero that this report dated 9/27/2024 documents three (3) Type A citations which shall be posted for 30 consecutive days as there are immediate risks to the health, safety, or personal rights of children in care. Also, LPA Reyes informed the licensee, Ms. Salguero to provide a copy of this licensing report dated 9/27/2024 that documents any Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. During the exit interview, the LICENSEE, Irma Salguero confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. During today's inspection, Type A and Type B deficiencies were cited under Title 22 Division 12. and Health and Safety Code.
