What the inspector wrote

This is a change of location, previous facility number is 406216951. This is a follow up visit to the initial inspection conducted on 4/4/25. On 4/11/25 at 10:55 AM, Licensing Program Analyst (LPA) Matthew Sapien conducted a follow up pre licensing/change of location inspection at the abovementioned residence. LPA met with Applicant Mayra Hernandez-Medina and explained the nature and purpose of the inspection. LPA observed zero day care children, however, the Licensee’s Husband and son were present (cleared and associated). This follow up inspection was conducted to assess safety and compliance. During today’s visit, LPA observed a retractable fence added to a portion of the backyard to separate the inaccessible area of the yard. In this same inaccessible area of the backyard, LPA observed tightly fitted straps added to a covered jacuzzi. Moreover, a Recreational Vehicle, or RV, parked in the driveway of the house, was thoroughly inspected. The Applicant attested and declared that the RV is owned by the homeowner and the homeowner’s spouse. A DMV document was provided proving vehicle ownership. According to the Applicant, no one currently resides in the RV and it will also be moved from the residence in the coming weeks. Applicant will inform the Department when the RV becomes moved. Based on today’s inspection, Applicant has corrected all the deficiencies issued from the previous inspection on 4/4/25. Pending Licensing Program Manager (LPM) review, the abovementioned home meets Title 22 Division 12 requirements of a Large FCCH license. LPM has read the report and per LPM, the Large FCCH license is effective on today's date, 4/11/25 A notice of site visit was given to Applicant and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Exit interview conducted and report was reviewed with applicant, Mayra Hernandez-Medina.
