(Continued, Page 2...) Director actively worked with pest control services, heating vent services, restoration services and carpet cleaning services within approximately 7 days after pipe bursting. Per director, children were temporarily combined in a classroom while third party services fixed pipe. Children's families and staff were notified of pipe bursting and notified of combination of classes. LPA found director to have addressed pipe bursting appropriately. Although the above allegation may have happened or is valid, based on LPA's observations, interviews and record review which were conducted, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with director, Michelle "Shelley" Marquez.
