What the inspector wrote

On May 6, 2026 at approximately 8:40AM, Licensing Program Analyst (LPA) Alvarado conducted an unannounced annual visit at the facility. LPA Alvarado upon entrance of the facility met with two parent volunteers and explained the purpose of the visit. LPA then met with Vickie Van Winke (S1) who is a teacher and the Resource Specialist at the facility. Present in the Facility is (S1), and two staff members supervising 12 Preschoolers. LPA observed the two staff members present to be fingerprint cleared and associated to facility number. Per S1, Staff 3 (S3), is a substitute teacher for both the preschool and elementary school. LPA did not observe S3 to have fingerprint clearance or association to facility number. A Type B deficiency is being issued for a staff member not having fingerprint clearance. Preschool program operates at Hillside Christian Academy in classrooms #1-6. Hours of operation are Monday through Friday 7:30am to 5:30pm. Upon LPA’s arrival, LPA requested to meet with facility director. Per S1, director left facility as of some time in 2025 and then next interim director approximately in 2026. Department was not notified that preschool program does not have a director or of any director change. A Type B deficiency is being issued for not meeting reporting requirements. Per S1, there is an interim director for the preschool program. LPA discussed director packet with S1. LPA inspected day care areas for health and safety hazards. In preschool component’s restroom, LPA observed accessible cleaning solutions and chemicals. LPA informed S1 that chemicals and poisons must be made inaccessible to children. A Type B deficiency is being issued for accessible poisons. During visit, LPA observed TK children and preschool age children to be co-mingling. Per S1, children in TK group are not preschool component children. LPA discussed age range of TK children with S1. LPA stated elementary school children and preschool children must be kept physically separate from one another. LPA also informed S1 that TK age children, when mingled with preschool component, are considered preschool age children. Continued On Page 2... Page 2 Continued... At approximately 11:25am, LPA Quimbo arrived to facility. LPAs reviewed facility records that included teaching staffs and children’s records. LPAs observed teaching staffs’ records to be incomplete. LPAs observed Mandated Reporter certifications for preschool component’s teaching staff to be expired and do not include the AB1207 certification. A Type B deficiency was issued. LPAs also observed staff files to be incomplete. Staff files did not include immunization records, staff qualification transcripts or licensing documents. A Type B deficiency was issued. LPAs observed children’s records to also be incomplete. Children’s records do not include required licensing documents nor admission agreements. A Type B deficiency was issued. This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. LPAs observed an enrolled child to be listed in program with a prescribed medication. LPAs did not observe medication nor medical records for prescribed medication. A Type B deficiency was issued. Refer to 809D for more information on deficiencies issued. Appeal rights were provided. During today’s visit, LPAs provided S1 with Entrance Checklist (LIC125), Records to be Maintained at the Facility (LIC311A), Child Cre Center Provider Staff Qualifications checklist. Annual Continuation will be conducted at a later time due to time restraint. Exit interview conducted and report was reviewed with Facility Representative, Vickie Van Winke and Interim Director Linda Huey.
