What the inspector wrote

On February 23, 2026, at approximately 3:00PM Licensing Program Analyst, (LPA) Katie Krenn arrived at the facility for an unannounced annual inspection visit. LPA met with Site Director, Teresa Thomas-Killeen (D1) and explained the purpose of today's visit. Present at the facility in addition to the director was a staff and 13 preschool children (ages 2 - 6 years old). The facility remains within its capacity limits and meets the required ratios for today’s visit. All staff present during the visit and staff listed on the personnel report have obtained criminal record fingerprint clearance from the Department of Social Services and are associated with this facility on Guardian. Program operates Monday through Friday from 7:300AM – 5:30PM, year round. Per D1, there are no pools, spas, or bodies of water on the property. LPA observed that required documents such as notification of parents’ rights, personal rights, and car seat laws were displayed and visible to the public. LPA reviewed the Entrance Checklist (LIC 125) with D1. LPA reviewed the files of five children, confirming that their emergency contact and medical information were complete and in order. LPA observed that three children were missing LIC 995 Notification of Parent’s Rights and two other children had partially completed forms. LPA reviewed these with D1. LPA reviewed two staff files and confirmed that both the director and staff have current CPR First Aid and Mandated Reporter Training Certificates. continued on page 2 Page 2 Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. To improve the quality and value of the new inspection process, a survey may be sent to the email address provided. Please complete the survey and share your inspection experience. This visit will be continued at a later date. A Notice of Site Visit was given and must remain posted for 30 days. The Director, Teresa Thomas-Killeen was unable to sign the report and LPA was unable to review the report, because the director was actively involved in supervising the children. Exit interview was not conducted.
