C1 = the child On May 20, 2026 at approximately 9:00 AM, Licensing Program Analyst (LPA) Tso conducted an unannounced, case management visit. LPA met with the Site Director, Cathy Kirkpatrick, and explained the purpose of the visit. Present in the facility is 5 staff, and 25 children in care. . The case management visit is regarding an unusual incident that occurred on April 22, 2026. Facility self-reported incident to CCLD on April 29, 2026. On April 22, 2026, C1 and other children took turns crossing the monkey bars. C1 slipped off and fell on C1’s left arm into the tan bark. C1’s parent picked C1 up 17 minutes. LPA interviewed with the Site Director. C1 had not been absent from the facility and returned to the facility next day, April 23, 2026. There was no sufficient evidence for any violations. There were no deficiencies cited at this time under CCR, Title 22, Div. 12. A copy of today’s report was given to the Director. Notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director, Cathy Kirkpatrick.
