What the inspector wrote

On August 27th,2021, Licensing Program Analyst (LPA) Kassandra Medrano conducted an annual required inspection. Analyst toured the facility building and grounds, conducted an evaluation of the physical plant, and reviewed some staff files, follow up visit to be made to complete file review. At 9am: During tour of facility, LPA observed a staff member not associated to facility. After review of records, it was confirmed that the staff did not have fingerprint clearance. Individual was immediately removed A $100.00 civil penalty per person per day up to 5 days (max $500 per person) for the first violation within a 12-month period will be assessed for individuals who do not have fingerprint clearances, are not associated to the facility, or have regular and routine contact with the day-care children. For Subsequent violations within a 12-month period, a civil penalty of $100 person per day up to 30 days (max $3000 per person) will be assessed for individuals who do not have fingerprint clearances, are not associated to the facility or have regular or routine contact with the day-care children.Days and hours of operation are Monday- Friday: 6:30am-6pm. The following was observed today: Current facility director is Minh Bui. Current rooms used for care Discovery Preschool, Preschool, and Pre-K. The following items were reviewed as part of today's visit: Care and Supervision of the Children, Child Discipline Procedures, Emergency Evacuation Procedures (smoke and carbon monoxide detectors present and in working order), Medication Policies, Isolation of Sick Children, Napping Requirements, Food Service, Transportation, Parents Rights, and Reporting Requirements. Posting requirements for site visits were also discussed as well as AB 633 requirements. Current forms and Title 22 Regulations can be obtained through the internet at www.ccld.ca.gov . Staff immunization are on file. Director was reminded that as of September 1, 2016, a person may not be employed or volunteer at a child care facility unless he or she has been immunized against influenza, pertussis, and measles or qualifies for an exemption pursuant to Health and Safety code 1596.7995 and 1597.662. Influenza Declarations were also reviewed. Director was advised of Pesticides training. For More information about changes to the Healthy Schools Act, templates, articles, and required training you can inspect the DPR website at: https://apps.cdpr.ca.gov/schoolipm/childcare/training/main.cfm . Director was informed about the Provider Information Notices (PINs) on CCLD website. Director was reminded of Mandated Reporter Training available on CCLD website. Training must be renewed every two years. Facility is clean and in good repair. California Code of Regulations, Title 22 deficiencies are being cited on the following page(s): "NOTICE OF SITE VISIT" DOCUMENT WAS POSTED ADJACENT TO THE MAIN ENTRY DOORWAY AND VISIBLE TO PARENTS. LICENSEE MUST POST ANY TYPE A DEFICIENCIES DURING TODAYS VISIT WITH THE NOTICE AND LICENSEE UNDERSTANDS THE NOTICE AND TYPE A DEFICIENCIES MUST REMAIN POSTED FOR THIRTY DAYS. REQUIREMENTS FOR AB 633 FACT SHEET AND A COPY OF ACKNOWLEDGEMENT OF RECEIPT OF LICENSING REPORTS (LIC 9224) WERE DISCUSSED WITH APPLICANT/PROVIDER. PROVIDER UNDERSTANDS THIS REQUIREMENT. This TYPE A citation page shall be posted for 30 days along with the \342\200\234Notice of Site Visit.\342\200\235 This page shall be provided to all parents of children currently enrolled and any future children being enrolled for the next 12 months per AB 633 requirements.
