On 5/13/25 at 10:30AM, Licensing Program Analysts (LPAs) Alvarado and Tse conducted an unannounced Plan of Correction (POC) for deficiencies that were cited during an annual visit on 4/8/2025. LPAs met with assistant Susan Lasat (S1) and explained the purpose of the visit. Present in the facility is (S1) and two assistants supervising a total of 4 children. At approximately 11:00AM, two more children arrived to the facility. All adults living or working in the facility have acquired fingerprint clearance and association to the facility. Licensee was not present during the visit. During an annual visit on 4/8/25, Type B deficiencies were cited in regards to operation of a family child care home, infant safe sleep, personnel requirements. LPAs observed a bookshelf that was not secured to the wall, LPAs did not observe any sleep logs for the infants in care, LPAs observed blankets in the crib, Chemicals were present in the facility bathroom accessible to children, and no staff present in the facility had valid CPR/First Aid. During todays visit on 5/13/25, five out of the eight citations were cleared. LPAs observed sleep logs being maintained for infants in care, cribs being free of loose articles and materials, furniture and equipment to be in good repair, chemicals were made inaccessible, and proof of enrollment for S1 in a CPR/First Aid course. Facility remains pending with three citations that were issued on 4/8/25 regarding Personnel Records and Operation of a Family Child Care Home. Mandated Reporter Training was not observed to be completed, Personnel Records were not observed to be maintained, and Licensee was not present in the home. No deficiencies were cited during today's visit on 5/13/2025. A Notice of Site Visit was provided and must remain posted for 30 days.
