What the inspector wrote

On 10/07/2025, at approximately 9:00AM, Licensing Program Analyst (LPA) Alvarado conducted an unannounced annual visit at the facility. LPA Alvarado met with facility director Karen Corzin(D1) and Explained the purpose for today\342\200\231s Inspection. Present in the Facility is (D1) and four staff members supervising a total of 13 children (Infants). The facility is operating within staffing and ratio requirements on this day. Staff present have fingerprint clearance and are associated as of today\342\200\231s inspection. This facility is a combination center, with a Preschool License (414002580) located in the same building. LPA along with (D1) inspected the facility for any health or safety hazards. Infants utilize the Infant Playroom, Infant Dining and Activity Area located on the main building and The Younger infant classroom located on the left sided of the building. LPA observed changing tables and running water at an arms distance. LPA Alvarado also observed cubbies for infant belongings. Facility also uses the Outdoor Play Yard which is a fenced outdoor play area designated for infant use. Outdoor play area has sufficient shading and has fake grass for safe landing zones. LPA observed the outdoor play areas to be free of debris and other loose articles. There are age-appropriate toys and equipment present. Each outdoor play area is enclosed by a fence that is at least four feet high. No Pools, hot tubs, spas, or similar bodies of water present in the facility. Facility Hours of operation are from Monday-Friday 7:30AM-5:00PM. Facility Provided Am Snack, Lunch and PM Snack. Facility uses Chefables who provides meal services and LPA observed a lunch menu to be posted. LPA Alvarado also observed infant bottles properly labeled and refrigerated. Facility also provides drinking water from a filter that is attached to the sink. Facility Provides napping equipment such as cribs and cots. LPA observed blankest on the side of the crib and reminded (D1) that infant cribs must be free from all loose articles and item. No infants were observed present in the crib and (D1) removed them. Safe sleep was re-addressed with (D1). LPA also observed infant pajamas that have movements of the Arms and Feet when an infant is placed in it. Facility has a washer and dryer and provides washing as needed. Per (D1) there are no weapons or firearms in the facility. Facility uses Brightwheel to sign in and out as well as another form of communication with families. Continued On page 2... Page 2 Continued... Facility Building is equipped with a fully charged fire extinguishers. LPA observed Duo Fire alarm systems and Carbon Monoxide Detector in the facility. Facility has a Fully equipped First Aid Kit. Electrical outlets are childproof and inaccessible. LPA observed Poisons, cleaning detergents, and other chemicals made inaccessible to children in care. Facility has child proof locks and has made off limit areas barricaded and inaccessible to children. LPA reviewed 2 Infant staff files and 5 Infant files. Infant files were complete and Facility document 15Min sleep checks on the brightwheel app. At least one staff members present has current Pediatric First Aid/CPR training that Expires 7/2027. Staff have qualifications and immunization records available for review. Staff have the completed Mandated Reporter Training on File that . Facility conucts Emergencry Drills that are conducted at least once every six months, last Emergencry Drill that was conducted was on 05/13/2025. All required documents to be posted in a prominent public accessible area at the facility were observed to be posted and accessible for review upon entry to the facility. LPA advised on a few that needed to be posted and observed (D1) post them. (D1) was provided by LPA Alvarado a packet containing the Recently Approved Safe Sleep Regulations in Effect PIN 20-24-CCP, along with a Copy of the Infant Sleep Chart for Documentation of the Infant 15-Minute check, a copy of the Infant Sleeping Plan (LIC 9227), CDSS additional information regarding Safe Sleep in Child Care information and a Copy of the Child Care Centers Regulations that include 101239. Fixtures, Furniture, Equipment and Supplies, 101416.5. Staff-Infant Ratio, 101419.2. Infant Needs and Services Plan, 101429. Responsibility for Providing Care and Supervision for Infants, 101430 Infant Care Activities, and 101439.1. Infant Care Center Sleeping Equipment. Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). Facility tested for Lead on 8/17/2022. Lead Results are posted on the California Water Board Site and Load Date Results were posted on 8/31/2022. Facility is in compliance with the (PIN 21-21.1-CCP) written directives. For IMS information see PIN 22-02-CCP. Based on today's inspection 10/7/2025, No deficiencies were issued today during LPA's visit. See LIC9102-TA for Technical Advisory regarding Physical Plant and Child Records. Appeal rights were provided to (D1). A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit Interview was conducted with Facility Director, Karen Corzin.
