What the inspector wrote

On July 15, 2025, at approximately 12:30 a.m., Licensing Program Analyst (LPA) Zeynep Basak conducted an unannounced visit for case management. The case management visit took place during the course of a complaint investigation. During the investigation, the LPA obtained pertinent documents, conducted observation, and interviewed staff members. Based on observations, record reviews, and interviews, LPA found that the incident, which occurred on 7/7/2025, had not been reported to the Community Care Licensing Division. A Type B violation will be cited on LIC809D, in accordance with the California Code of Regulations, (Title 22, Division 12, Chp 1). The report was reviewed and signed by the director, Christopher Geno. An exit interview was conducted, and a Notice of Site Visit was issued to remain posted for 30 days. Appeal rights were discussed and provided to the director, Christopher Geno.

(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1).Supervision shall include visual observation. Based on observations, record review, and the interviews conducted, this requirement was not met, as evidenced by the child left the facility without supervision. This poses an immediate health and safety risk to children in care.
