What the inspector wrote

On 9/15/2025, Licensing Program Analyst (LPAs) Wadhwa and Garcia conducted a case management inspection and met with Director Noel Trevisan. LPAs explained the purpose of case management visit, which was to cite deficiencies observed during today’s inspection. LPAs inspected facility for health and safety hazards. At 2:50 PM, Based on record review, interview, and observation, LPAs confirmed there were 31 school-age children present in Room 1 being supervised by two staff. LPAs observed the facility is operating over capacity. Based on today’s inspection, a Type A violation has been issued for over capacity in accordance with the Title 22 Division 12 of California Code of Regulations. See LIC 809D. LPAs informed Director, Noel Trevisan that this report dated 9/15/2025 documenting Type A citation(s) shall be posted for 30 consecutive days as there is immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPAs informed the Director, Noel Trevisan to provide a copy of this licensing report (9/15/2025) that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed acknowledgement of receipt of Licensing Report (LIC9224), or other written statement, must be placed in the child’s file for verification. A copy of this report with the appeal rights was provided, and signature of this form acknowledges the receipt of these documents. Exit interview conducted and report was reviewed with Director, Noel Trevisan. Director was advised Notice of Site Visit to remain posted for 30 days.
