Correction check·October 10, 2025

On 10/10/2025, at 1:10 PM, Licensing Program Analysts (LPAs) Wadhwa and Ly conducted an unannounced Plan of Correction (POC) Visit and met with Site Director, Noel Trevisan. The purpose of today’s visit was explained. Present during the visit were 4 staff including the Site Director. There were no children present during today’s visit. All adults present today have finger-print clearance on file. The POC is regarding a Type A deficiency cited on 9/15/2025 for operating in violation of the Teacher-student ratio requirements. On 10/6/2025 at 9:09 AM, Site Director emailed LPA Wadhwa with a written plan of correction that included measures the facility would undertake to remain in compliance of the Teacher-student ratio requirements. LPAs reviewed and discussed the written plan with the Site Director. LPAs inspected the facility for health and safety hazards and based on inspection and discussion with the Site Director, LPAs have cleared the Type A deficiency that the facility was cited on 9/15/2025. LPAs provided a letter of clear deficiency to the Site Director. A copy of this report with the appeal rights was provided, and signature of this form acknowledges the receipt of these documents. Exit interview conducted and report was reviewed with Site Director, Noel Trevisan. The Site Director was advised Notice of Site Visit to remain posted for 30 days.
