101215.1:Child Care Center Directors Qualifications and Duties: (b) All child care centers shall have a director.This requirement is not met as evidenced by: Based on observation, interview, and record review, the facility did not comply as the facility does not have a director for this facility as of January 23rd, 2026. This poses a potential health, safety or personal rights risk to persons in care.

101216.2:Teacher Aide Qualifications and Duties:(e)An aide shall work only under the direct supervision of a teacher.This requirement is not met as evidenced by: Based on observation, interview, and record review, the facility did not comply as the facility has aides working without the direct supervision of a fully qualified staff. LPA observed there are two classrooms where there were only aides supervising the children. This poses a potential l health, safety or personal rights risk to persons in care.

This poses an immediate health, safety or personal rights risk to persons in care.

101516.5: Teacher-Child Ratio:" (b) There shall be a staffing ratio of one teacher and one aide present to every 28 children in attendance.(1) A teacher shall supervise no more than 14 children or with an aide a maximum of 28 children."This requirement is not met as evidenced by: Based on observation and interviews, the facility did not comply with the section cited above as LPA observed that there were 31 school age children with two teacher aides without the supervision of a qualified staff. This poses an immediate health, safety or personal rights risk to persons in care.
