TEACHER AIDE QUALIFICATIONS AND DUTIES
101216.2:Teacher Aide Qualifications and Duties:"(e)An aide shall work only under the direct supervision of a teacher."This requirement is not met as evidenced by: Based on observation and record review, the facility did not comply with the section cited above as upon arrival, LPA observed that there were two aides supervising 29 school age children without the direct supervision of a fully qualified teacher. This poses an immediate health, safety or personal rights risk to persons in care.

This poses a potential health, safety or personal rights risk to persons in care.

(3) The licensee shall require each recipient of the licensing report described in paragraph (1) pertaining to a complaint investigation to sign a statement indicating that he or she has received the document and the date it was received."This requirement is not met as evidence by: Based on observation, interview, and record review, the facility did not comply as the facility still has to give the LIC 9224s for some children and the newly enrolled children from Allen Site. This poses a potential health, safety or personal rights risk to persons in care.
