What the inspector wrote

On March 10, 2026, at approximately 1pm, Licensing Program Analyst (LPA) Tapia-Mandujano conducted a Plan of Correction unannounced visit to the facility. LPA met with Site Director, Gesmyne Bell, and explained the purpose of the visit. Present during the inspection were a total of three champion staff and four Child Care Careers (CCC) substitutes supervising a total of 74 school age children. LPA observed and verified all staff through Guardian roster and found that all Champion staff present are associated with the facility. LPA has cleared the deficiency. During inspection, LPA started reviewing children’s files to ensure that each child has the acknowledgement of Receipt of Licensing Report (LIC 9224). Based on observation, file review, and interview, the facility is still getting this form from families. LPA will not clear deficiency today. Site Director must submit all the completed acknowledgement of Receipt of Licensing Report (LIC 9224 with tracking log to ensure that the forms are completed by March 13, 2026. At approximately 2:55pm, LPA started the health and safety inspection. During today’s inspection, LPA observed that each classroom is within appropriate teacher to child ratio. LPA has cleared the deficiency. During file reviews of reviewing qualifications, LPA observed that the TK/Kinder classroom was being supervised by two teacher aides. This a repeat violation that will result in a Type A deficiency and civil penalties were assessed. LPA observed that the reports from 2/11/26, 2/20/26, 2/25/26 were posted. LPA also observed the Noncompliance Conference Report from 3/04/26 to be posted. LPA will clear deficiency. Continued on Page 2... Page 2 Continued... During inspection, LPA observed that the facility has implemented new policies to help with maintaining supervision. Per Site Director, the facility has carts with children’s materials so that the children do not have to run inside for their belongings. Per Site Director, when children are outside, the children will use the bathrooms outside to avoid them coming into the classroom unsupervised. LPA will clear deficiency. During today’s inspection, LPA provided clearance letters for the above deficiencies to Site Director. LPA Tapia-Mandujano informed acting Site Director, Gesmyne Bell that this report dated 3/10/26 documents one Type A citations which shall be posted for 30 consecutive days as there are immediate risks to the health, safety, or personal rights of children in care. Also, LPA Tapia-Mandujano informed acting Site Director, Gesmyne Bell to provide a copy of this licensing report dated 3/04/26 that documents any Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Deficiencies were observed today under CCR, Title 22, Div. 12, Chapt. 1. A notice of site visit was given and must remain posted for 30 days. Exit interview was conducted and report was reviewed with Site Director, Gesmyne Bell.

This poses an immediate health, safety or personal rights risk to persons in care.
