What the inspector wrote

On January 23, 2026 at approximately 8:00AM Licensing Program Analyst, (LPA) Katie Krenn arrived at the facility for an unannounced case management visit. LPA met with the Site Supervisor, Olga Martinez Chavez (D1) and explained the purpose of today's visit is to follow up on the recent incident that the facility self reported, where a child was briefly left without supervision. Present at the facility were five staff supervising 11 preschool children (ages 3-5 years old) and four toddlers (ages 2-3 years old). This is a Title V program. The facility remains within its capacity limits and meets the required ratios for today\342\200\231s visit. All staff present at the facility during this visit have obtained criminal record clearance from the Department of Social Services. Program operates Monday through Friday from 7:00 AM - 5:45 PM year-round. Per D1, there are no pools, spas, or bodies of water on the property. LPA observed all the required postings in both classrooms in both English and Spanish. The facility had a first aid kit, a smoke detector, a fire pull alarm, a charged fire extinguisher, two carbon monoxide detectors, and an emergency disaster kit. LPA tested the carbon monoxide detectors that had been installed since the previous inspection. LPA found both of the new carbon monoxide detectors to be working properly. This facility uses an app for signing children in and out. LPA reviewed the sign in and sign out files and found that all the children present had been signed in. Each person designated to pick up children has their own unique pin. LPA conducted and interview with D1. The incident involved a toddler that was able to enter an empty classroom as a family was leaving the classroom, because the door was not latching properly when closed. The child was unsupervised in the classroom for approximately 30 seconds, before a teacher found him. continued on page 2 Page 2 The facility had physical bells on the inside of the doors and could hear the them from inside the classroom, but not outside the classroom. Per D1, the door was fixed on January 9, 2026. D1 also had electronic bells added. LPA tested both doors and observed that the electronic bells could be heard well from inside the classroom. LPA had the facility move the physical bells to the exterior of the door, because the electronic bells were less audible outside. LPA tested the doors to ensure that they were closing properly. LPA tested the door after the physical bells were moved to the outside of the door that parents use to pick up and drop off children. LPA was able to hear the bells from outside the classroom. Facility Representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
