What the inspector wrote

On November 25, 2025 at approximately 12:50pm, Licensing Program Analyst (LPA) Catrina Quimbo conducted an unannounced, annual inspection. LPA met with licensee, Armida Isabel Tran, and explained the purpose of the visit. Present during LPA's visit included licensee, licensee's adult daughter/assistant, and 5 enrolled children (2 infants and 3 preschool age children). Licensee is operating within capacity limits and ratio during LPA's visit. Licensee lives in the multi-level home with their spouse, adult daughter, and one minor child. All adults living in the home have fingerprint clearance on file. Licensee stated they are aware children under 10 years old living in the home are counted towards their total capacity. Hours of operation are Monday through Friday 8:00am to 5:00pm. With licensee, LPA toured day care areas for health and safety hazards. The DAY CARE AREAS are the lobby area (for diapers and cubbies), hallway bathroom in lobby area, nook area (dining area), family room (main play area), portion of sun room (napping area), front yard and backyard. The OFF LIMIT AREAS are portion of sun room, garage, master bedroom, bedrooms #1 through #3, dining room, kitchen, and living room. The portion of the sun room that is off limits is made inaccessible with a child safety gate. Access to remainder of off limit areas are through closed doors that are locked. LPA observed day care areas to include a variety of toys and materials that are age appropriate. Flooring in day care area includes soft padding that is in good condition. LPA observed cleaning solutions and chemicals to be made inaccessible in home's high shelves. There is an age appropriate play structure in the main play area that is in good condition. Home is equipped with a fully charged fire extinguisher, emergency backpack, smoke alarms, and a carbon monoxide detector. LPA tested carbon monoxide detector during today's visit which was observed to be in working condition. (Continue Report on Page 2...) (Continued, Page 2...) Bathroom for children's use is located in the lobby area. LPA observed bathroom to be equipped with age appropriate toileting equipment. LPA also observed bathroom to be in working condition. There is a diaper changing table in the lobby area. Diapers and wipes are provided by children's families. LPA observed diapers and wipes to be stored appropriately, labeled with children's names. Licensee disposes dirty diapers in garbage bin with a tight fitting lid. Children sleep in the main day care area on cots. Sheets and blankets are provided by children's families and are sent home weekly for laundering. Cots are stored appropriately when not in use. Licensee stores children's sheets in cubbies so that they are not touching each other. Infants sleep in cribs in the sun room. LPA observed a crib in sun room to have a mobile attached to the side of the crib, and hanging over the crib. LPA reminded licensee nothing can be attached to the crib or hang over the crib. A type B deficiency was issued. Licensee removed mobile from crib during LPA's visit. Deficiency was cleared. LPA observed infant present to fall asleep while being bottle fed. LPA observed licensee's daughter to move infant to a crib right away once infant fell asleep. LPA observed infant to sleep in a crib, free of loose articles and objects. Sleeping logs are maintained for napping infants through an electronic application. LPA reviewed random months of sleeping logs from September through November 2025. LPA observed sleeping logs to document the initial time the infant falls asleep, the time the infant wakes up, the 15 minute time stamp of when the napping infant was checked on while sleeping and the sleeping position of the infant. Licensee also has a completed Individual Infant Sleeping plan form (LIC9227) for infant under 12 months. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Children play in the front yard and backyard. Both yards are entirely enclosed and fenced. LPA observed outdoor areas to include a variety of outdoor toys and materials that are in good condition. There are swings and a play structure with resilient padding underneath. Per licensee, outdoor toys and materials are washed weekly. LPA did not observe any pools, spas or bodies of water on site. Licensee provides a food service that includes breakfast, lunch, and snacks. Reusable dishes and utensils are utilized and sanitized daily. (Continue Report on Page 3...) (Continued, Page 3...) LPA reviewed facility records that included all enrolled children's records. Children's records are complete with emergency information and immunization records. LPA reviewed licensee and daughter's records. Licensee's CPR certificate has expired as of 08/2025. Per licensee, their CPR renewal was conducted 11/22/2025, however, has not received their certificate as of this date. Licensee provided LPA proof of their registration directly from the CPR trainer. Licensee will provide proof of completed certificate to LPA. Licensee and daughter have proof of their immunization records that were made available for LPA's review. Licensing documents are posted in the lobby area. Emergency disaster drills are conducted at least once every six months. Last documented drill was conducted 6/8/2025. A childcare roster is also maintained and was made available for LPA's review. Per licensee, there are no weapons or firearms in the home. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the Licensee Armida Isabel Tran, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. A notice of site visit was given and must remain posted for 30 days. Appeal rights were provided to licensee. Exit interview conducted and report was reviewed with the licensee, Armida Isabel Tran.
