On April 17th, 2025 at approximately 12:05pm, Licensing Program Analyst (LPA) Tapia-Mandujano arrived at facility for a Case Management-Incident. LPA met with new Director, Claire Brady. The purpose of the inspection was to discuss the Unusual Incident Report that was reported to the department on February 4th, 2025. Present in the facility is 27 preschool age children supervised by 4 staff. All staff present during today\342\200\231s inspection have fingerprint clearance and are associated to the facility. Facility operates Monday-Friday from 7:30am-5:30pm, year-round with periodic breaks. Facility is a combination center with an Infant license. Facility is licensed in 4 classrooms and currently are operating in 2 classrooms (Owls and Eagles). During today's inspection, LPA conducted a health and safety inspection indoors and outdoors and interviewed Director about the incident reported. The facility self reported an incident that occurred on February 2 nd , 2025. The incident involved a child being left unsupervised inside the classroom. Per interview conducted with Director, the children were outdoor playground in the afternoon. Per Director, a child needed to use the restroom, and one staff opened the door to the classroom to let the child in to use the restroom and stood outside holding the door open. Continued on Page 2... Page 2 Continued... Per Director, staff holding the door, turned head to help problem solve with another child and did not see another child (C1) go into the classroom. Director stated that staff turned her head back and guided child who she let in the classroom to wash hands, and child came back outside with the rest of the group and the staff closed the door. Per Director, another staff mentioned that she did not see C1 on the yard and immediately realized that C1 was inside the classroom. Per Director, the child was inside the classroom with the door close for less than a minute. Once the incident occurred. Staff came to tell Director about the incident. Per Director, she spoke to the staff that were present to find out what happened. Director also notified the parents of C1. Director also spoke to all preschool staff about the incident and the importance of doing sweeps and being aware of the door, especially as picks up are happening during this time. Deficiencies were issued today under CCR, Title 22, Div. 12, Chapt. 1. A Notice of Site Visit was given and must remain posted for 30 days. An exit interview was conducted, and report was reviewed and provided to Director, Claire Brady.
